What are your office hours?
The Sports Medicine Institute’s office hours are 8:30 a.m. to 5:00 p.m. Monday, Tuesday and Thursday and 8:30 a.m. to 4:30 p.m. Wednesday and Friday.

Where are you located?
Our office is located at 1601 Medical Drive in Pottstown. Our telephone number is 610-705-3055.

What hospitals are you affiliated with in the area?
Dr. Cooper is affiliated with Pottstown Memorial Medical Center, Paoli Hospital and Phoenixville Hospital Surgical Center at Limerick.

What type of insurance do you take?
The Sports Medicine Institute (SMI) participates in most managed health care plans and private commercial insurance including Aetna/US Healthcare products, Keystone Health Plan Central, Keystone Health Plan East, Personal Choice, Keystone 65, other Blue Cross programs, Cigna, and United HealthCare. SMI also participates in Medicare and numerous Workers’ Compensation programs.

If you are unsure if SMI participates with your insurance, please call your health plan to confirm this information.

What do I need to bring with me on my first visit?
On your first visit to SMI please bring your insurance card and a referral or referral authorization number from your primary care physician, if required. Also, it is extremely important that you bring any x-rays, MRI results or other pertinent test results with you when you come for your appointment. This information will help Dr. Cooper assess your condition in a timelier manner.

How do I know if I need a referral from my primary care physician?
Referral requirements are not the same for all insurers and health plans. In most cases, you need a referral only if you are insured by an HMO or Point-of-Service (POS) plan. These types of health plans usually require referral prior authorization for visits to specialists or for certain types of procedures. If you are unsure if you need a referral, check your benefits booklet or contact your benefits administrator or health plan. Most traditional and PPO plans do not require referral authorization.

What do I need to do if I have to cancel my appointment?
Appointments need to be cancelled 24 hours in advance in order to avoid a charge for the missed appointment.

What should I do in the case of an emergency?
Dr. Cooper is available 24 hours a day, seven days a week. If a medical emergency arises after business hours, please call our office. The answering service will page Dr. Cooper, and he will return your call as quickly as possible.
