Office Policies
- Copayments are expected at the time of the visit or the appointment will be rescheduled.
- There is a $20.00 no show fee for all missed appointments.
- There is a $10.00 fee for all disability forms that need to be completed. All forms are completed in the order they are received and generally take 3-5 business days.
- NO MEDICATIONS will be refilled over the weekend!
- Any person under the age of 18 must be accompanied by a parent of guardian at the time of there appointment, or the visit will be rescheduled.
